The flag's ratio is 1:1. It is an armourial flag (or banner of arms).

Description of coat of arms:
The shield is parted per fess. Above in a red field is a silver horse forcene. Beneath the golden field is charged with a red fess, superimposed by three golden cogwheels ordered per fess.
Meaning:
Twistringen had been a condominion in the Middle Ages, thus control often changed between the Bishops of Münster and the Counts of Hoya. The horse is alluding to the Welfen-kin, the successors of the Counts of Hoya. The golden field with red fess displays the arms of the Bishopric of Münster. The cogwheels as a distinguishing mark are symbolizing local industries and crafts.

According to Stadler 1970 the arms were granted by Oberpräsident of Prussian Hannover Province in 1936. On occasion of the elevation of Twistringen to a city the armourial flag was granted in 1964.
