Hazel Park is an unincorporated lakeside community located on Lake Conneaut in Crawford County, Pennsylvania.
The flag is a dark blue field with a ratio of 2:3. In the center is a steering wheel from a ship. Above the wheel is the community name, with the lake the community sits by under the wheel. on either side of the wheel is a white box with a compass rose star above it, and waves underneath. Inside the white box are words which say the year the community was established, 1948.
