It is a typical Portuguese communal flag, with the coat of arms centred on a plain yellow field.

The arms are Azure a Nereid ( a kind of mermaid, a daughter of ancient Greek God Nereus) Or. Mural crown Argent with three visible towers (village rank) and white scroll reading in black upper case letters "ALVARELHOS - TROFA".
Meaning:
Alvarelhos is home to a Bronze Age castrum, whence the nereid imagery might have come from, look here.

The same like version 1999, but the inscription of the scroll had been "ALVARELHOS - SANTO TIRSO".

Alvarelhos Commune was one of the pre-2013 communes of Trofa Municipality (q.v.); it had 3151 inhabitants in 2011 and covered 6.5 km².

Shield Azure with base wavy of Argent and Azure, issuant from base a demi wheel Argent of a watermill, in centre two grapevines Or leaved Argent ordered per fess, in chief a Paschal Lamb passant Argent. Mural crown Argent with three visible towers (village rank) and white scroll with inscription in black initials "GUIDÕES".
