It is a fairly typical Portuguese communal flag, with the coat of arms centred on a on a black and red quartered background (town rank).

The arms are Argent a railway handheld stop signal Gules rimmed and handled Sable per pale between two corn cobs Or leaved Vert and in base two barrulets wavy Azure. Mural crown Argent with four visible towers (town rank) and white scroll reading in black upper case letters "ALFARELOS".
Meaning:
The railway handheld signal in the arms stands for the eponimous railway station, located in the neighbouring commune Granja do Ulmeiro (also with railway themed arms)
