It is a typical Portuguese communal flag, with the coat of arms centred on a plain yellow field.

Shield Azure, a cow passant Proper (= Argent spotted Sable), surrounded by a wreath of vine Or, fruited and leafed of the same, in chief a heart Or, flaming and pierced by a sword Argent. Mural crown Argent with three visible towers (village rank) and white scroll with inscription in black capitals "CRIAÇÃO VELHA - MADALENA".
Meaning:
The pierced heart is an attribute of Our Lady of Sorrows, the local patron saint.
