I couldn't find it there any more, but if I got that right, it would be

It reads something along the lines of:
1: The flag of the municipality and city of Lubomierz is a rectangular
cloth with its width (height) to length in a ratio of 5: 8
2: It consists of two horizontal stripes of equal width (height) of the
following colours: the upper stripe yellow, the lower stripe green.
3: A image of the flag is attached as Annex 1.
