According to Roberto Breschi's presentation titled: "The
Small Italian States Which Lasted Beyond 1700" at ICV 19
(York, 2001), the Republic of Cospaia used the traditional flag -
horizontal with four Venice-like teeth, divided diagonally from
low hoist to top fly (without accounting the teeth). The upper
(hoist side) part is black and the lower part and teeth are
white.

Cospaia was a small republic in Italy that came about by error. In 1440, Pope
Eugene IV sold some land to the Republic of Florence. An error excluded Cospaia
in the sale. The Papal State believed the territory was gone, and Florence made
no claim to it, so the inhabitants declared independence. Cospaia was
independent until 1826, when the Papal States and the Grand Duchy of Tuscany
divided it between them.
