The flag of the Croatian Hockey Federation (photo) is defined in Article 10 of the Federation's Statutes, last amended in 2010 (text) as vertically hanging, dark green - of the colour of grass, with the Federation's emblem in the to left corner.
The Federation's emblem is a white circular emblem outlined in yellow with two sticks issuing diagonally one from each side, one with a red and the other with a blue head, thus encircling a yellow outlined white ball in the centre. The Federation's name is inscribed in yellow along the emblem's top and bottom.
