The following link leads to an image of the car flag purported to be the
one to have been used by General Crerar, the general officer in charge of the
First Canadian Army in 1944-1945:

It is based on the car flag used by a British commander in chief or
commander of a command but defaced with a gold maple leaf.
I have been unable to find if the flag was used before 1944 (the army was
formed in 1942). The army was disbanded at the end of the war and doesn't
appear to have been used by any other army commanders subsequently. Despite
the name, the army was not formed solely of Canadians (these made up less
then 50% for most of its existence) but rather was named thusly due to being
lead by a Canadian headquarter.
